Along with the required fees due at time of enrollment, the following forms must be filled out:
- Signed Tuition Agreement
- Enrollment Form
- Personal History Form
- Health Statement with Immunization Record
- Emergency Release Form
- Child Release Policy
- Acceptance of "FMA Student Policy Manual"
A copy of your child’s birth certificate will also be needed for our files.
